A relative of victims of American Airlines flight 587, which crashed in New York in November 2001, holding a picture of the victims looks at projected slides during the US government investigators' report to the National Transportation Safety Board (NTSB) in Washington 26 October 2004. The report blamed pilot error for the crash of the Airbus A300-600 heading to the Dominican Republic shortly after takeoff from New York's JFK airport 12 November 2001 in which all 260 passengers and crew were killed as well as five people on the ground.
